	Subject of Books Note: audiovisual media cannot be checked out on a Teacher Card	Type and Number of Books Requested (max 30). For example: nonfiction 10, fiction 20, picture books 10. Any other information that will assist us.
	What are you needing the books for? E.g. going to zoo, science fair projects, teaching a specific subject through story and fact books. We can better develop the collection for you if we have details about how you plan to use them.	Specific titles needed? Please include the author or titles below.
	Date Needed (a minimum of 2 days is needed to fill your request) * Allow at least 2 days from today.	Need help? Give us a call during business hours, 303-441-3099 or use our Ask a Kid's Librarian form.
